Upload images to file and link in mysql database

I have a form of which I added two field to upload images. I also have a mysql db table with two fields for image1 and image2 and a directory on the server - uploads. I need someone familiar with PHP and PDO to complete the programming of the form filed to upload the images to the uploads directory and the path to the images to be inserted into the db table. If award this project you will be give access to a development server with the necessary files. The form files are complete except for the 2 image fields configuration, the database table is complete with the 2 images fields and the uploads directory on the server is in place. And the submit script is a PHP PDO format needs to have the image information added. Only the programming connection needs to be done. Also there is a page to view the form fields after inserted in the database and that would need to be configured to display the images on the page. This project needs to be done today so please do not bid if you can not finish today or within a few hours.
