Web-Based User Signup

Requirements: I have developed a data structure for an application I am buildling and I require a Web-based signup interface and script written in html/**perl**. I am capable of programming this component myself but don't have the time. The structure is quite simple and I will provide more details to the winning bidder. Each customer may have one ACCOUNT. Each ACCOUNT may have more than one with more than one SERVICE, and more than one NOTIFICATION PROFILE. Each NOTIFICATION PROFILE may have multiple actions associated with it. Each of the capitalized words above is a table within the MySQL database. None of the tables has more than 12 fields. I have a couple of requirements outside of getting the data into the table (user interface requirements), but I suspect these will be simple to accommodate. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased. ## Platform RedHat Linux 7.3
