Website + Convert Excel into Server Entry Form

We are signed up with Yahoo Business/Store to host our site. But, we need a basic website created for a loan modification company. Just need 4 to 5 pages. Also have a complex Excel Spreadsheet that will need to be converted to HTML or PHP...or other to allow users to enter values. Spreadsheet will perform various calculations instantly and user will be able to view results immediately. The Excel Spreadsheet is similar to a mortgage calculator except this spreadsheet has many input data cells required. Also, need SEO done on the site and various marketing to attract targeted visitors. I understand this portion may require constant monitoring and updating. Therefore, looking for someone to maintain this site monthly in order to sustain and increase traffic. There is very little programming involved. Only need the Excel Spreadsheet to be password protected. Users will have very temporary access to the Excel Form then the password will expire (within 30 days). Please provide quote for each task and combined task. Would like to see samples as well. I'm very easy to deal with as you can see in my ratings. I am also open to suggestions and advise.
