I would like to make a .net application that when run, shows the following fields, once the report it finished, the top half will allow me to edit the fields and the bottom half will show the results. Once the results have finished, the option to save as a text file or html file and also an option to Print the information below once all filled out.

The source code must be supplied, as I have stated this will be ongoing work. This must be done in VB.NET. I use Visual Studio 2015 Enterprise.
