Need a simple PHP form with checkboxes that passes values to the next page.

This is simple for those of you that know how to do it. I just need a working example of it to continue what I am working on. I need a form with some options as check boxes. After selecting one or more options, pass those values to the next page where they are put into a variable as comma separated values. For example: Form page with 4 check box options: Tucson, Phoenix, Flagstaff, Yuma. After checking Tucson and Yuma and clicking submit, you go to the next page where a variable called $cities would get assigned the value 'Tucson,Yuma'. Again, this should be very simple and quick for most of you experts. I don't need to provide you access to my cpanel or anything, just get me a working example of this.
