I checked the project description carefully, and please know Wordpress is our main area of expertise, we have large experience in creating custom themes and custom plugins.
I have worked with a lot of forms plugins, most known are gravity form and contact 7.
We have installed the GDPR for around 50 sites for an agency in UK, we did that couple weeks ago before 25th May when the lawy started to apply.

I would suggest adding as well a plugin that will notify the user that the site i using cookies and to ask their consent. ( like you can see in the site above in the footer ).
