Add functionality to Laravel 5 project

I have an existing project that currently features Laravel auth, profiles, basic discussion board. I have views created and wireframes available for the functionality I need added. I would like the following functionality to be built... The ability to 'check-in' daily with numerical, boolean and text-data (fields already decided). There are approximately 10 fields. A page where 2 parameters can be passed through eg. /insights?field1=calories&field2=sleep and a list of each of the stats will be returned with the item data and date from the check-in (I will later load this in to a graphing API myself to plot them both on a graph). I would expect check-ins to be in their own table with each item as a column. It must be simple to extend the check-in fields in the future. It must be possible to use a date picker to select any date for the check-in. There needs to be an easy way to send people to todays/yesterdays check-in. It must be possible to select a date that is already completed to edit the check-in with the existing data pre-populated. Other features also planned are an awards system and other quizzes, but I would like this done well first! This app will contain health data so good security practices is a must. I understand basic PHP and Laravel and plan to extend this code myself, so code must be clean and well documented. Good English is important. I will not consider any applications unless they contain 'I have read the project brief' on the first line. Many thanks.
