Very small business project for a non-profit organization

This is an application to be built for a non profit organization. The deadline fo the project is 4 days. There is no exception for the deadline. The project is simple, and needs work on the application forms, which are already exampled. For more detailed info look at the attached document. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Exclusive and complete copyrights to all work purchased. (No GPL, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site). ## Platform Should be able to work on all major platforms. Must be compatible for upgrading to SQL server in future.
