Need Business Card Template Changed

I need the attached file updated so that it reflects [login to view URL]'s printing requirements (see:? <[login to view URL]>). I need to be able to upload the file you provide to [login to view URL]'s website and be able to change the name, address, phone, fax, and email address section of the card on their site prior to printing. As the file stands, it's sizing is off and the image/design gets distorted when trying to upload to VistaPrint.com. The attached Zip includes 2 other stationary files that can be ignored (envelope & letterhead). In addition: "Mov" should be changed to "P:" "Fax" should be changed to "F:" The back of the card can/should be deleted/removed
