Wordpress (WEPAPP) expert wanted for digital contracts forms

We are looking for a WordPress expert to make a WEBAPP with smart contract forms. The forms are needed: 1) Registration form for an athlete. A. Athlete information (can be major or minor). B) Must generate a registration document in excel and PDF. C. The document generated after registration must be automatically mailed to BBA (More information to chat). 2) Registration form and contract. A. This form must be filled in with the data from the previous form and additional questions. (This form must include conditional fields) B. An electronic signature field must be included at the end. C. Once form 2 is completed, a DOC and PDF document should be generated with the added data and should be sent to a specific address. Important data: × Form 1 is addressed to a third party website. × It is important that the document generated by form 2 includes the signature of the person who filled out the form on the form. × It is important to autocomplete the fields with data that are redundant (names, date of birth, address, etc...).
