First thing first....what order is the ex-employee placing and on what system exactly? I presume this system is running on Linux. Have you checked all open ports and if the ex-employee has remote access to this machine? Is it at random intervals or at a particular time? I would start by checking that machine for any backdoors or rootkits. Kindly contact me so I can get more details on the system setup. The person who is entering the server, may be doing it by several ways, should review and block those routes, and then change all the access codes to the server or any site reacted. There would be multiple ways to fix this. Mainly, I'd start with logging. We need to see exactly what he's doing, to begin with. Also, making sure that you have proper logging turned on. This would require looking at your existing software and how it's setup. Once we see how and what exactly he's doing - we should patch that up, block his IP permanently, and go from there. (He could always use a VPN to get a different IP). There's other things I'd want to tighten up on the server as well. But that'd be a lot more detail.
