Need timesheet jobber

Fairly simple project. I need a basic employee timesheet (see attachment) where a user enters his normal working hours. The most important thing is that I need to be able to edit the drop down menu and the Total Hours box automatically adds up the Normal Hours and Overtime Hours boxes. The form then needs to be submitted via email. This need to work on a Wordpress page.

We want to create a timesheet - similar to this one: but in an editable PDF form that can calculate daily and weekly time based on military time. We would to to expand the functionality of the script to accomodate more employee information fields and to expand the reporting capability of the script.
