Form component in WordPress Website

We have a current website [login to view URL] and we would like to add the following functionality.
Website is built with WordPress so Wordpress experience is compulsory.
A new link in the main menu: "Comp Card Maker"
Once clicked on the link it will land a page where users will see a simple form
Sign up with facebook / Instagram (Social media signup) Social media plugin should extract some basic information which user should be able to edit.

Skin Colour: (Dropdown) (Pale, white, tanned, brown, dark) *
Height: (Dropdown) - (155cm, 156cm.... 195cm) *
Hair Colour: (Dropdown) - (Black, Grey, White, Golden, Blonde, Brown) *
Hair Length: (Dropdown) – (very short, short, shoulder height, long, very long)
Eyes Colour: (Dropdown) – Amber, Blue, Brown, Gray, Green, Hazel, Red and violet *
Dress Size: (Drop down) – (UK 4, UK 5 …. UK 32) *
Foot Size: (Drop Down) – (UK 4, UK 4.5, UK 5, UK 5.5 …. UK 12) *
Cup Size: (Drop Down) – (A,B,C,D,E,F,DD,DDD – Custom) *
Update photos: * (Allow uploading exactly 5 photos)
• User can upload 5 photos from their device (PC/Mobile photos)
• User can select 5 photos from social media (facebook or Instagram)
A “preview” of uploaded photos (150px width / adjust height automatically) should be shown right on the form after user have uploaded or selected them from social media

Once form is submitted user will be sent to next page where they will see their entered details and a link to download the comp card in pdf format.
Back end – All this information should be stored in database with following 2 facilities.
• Admin should be able to see the form entries including photos.
• Admin should be able to export the list in csv format (excluding photos)
• Admin should be able to email the user their comp card in pdf from admin interface.
