(50 euros) integration a new skin in Gravity forms (woocommerce) + upcoming projects

Hello,
There is a new skin I need to integrate in woocommerce order process form (through Gravity forms). The mockup is already made in illutrator (you may have the HTML or SVG code).
The work is mostly already done, all we need is to integrate the mockup,(however if you have an idea for improvement, I remine open).
Please find attached the screenshot with the explanations.
The .ai will be sent to you later if needed.
The work has to be done in max 24 hours.
The price is up to 50 euros.
I’m also working with some other clients..they will also be upcoming projects, if your work is smooth, I will be happy to establish a long term collaboration with you (if you have a team, it will be better, in terms of time optimization…).
