Website Enquiry Form

We are looking to have an advanced contact enquiry form built for our website.

• Name

• Contact Number

• Email

• 1st Line of Address

• Postcode

• Dropdown box 1 – Make

• Dropdown box 2 – Appliance

• Dropdown box 3 – Fault

• Additional message – Limited to 200 characters inc spaces

The list of faults will depend on the selection made in the Dropdown # 2 (see attached spreadsheet)

We also need this whole form to be sent in a format that can be printed off onto A5 sheet of paper directly without cut and pasting so I assume it will have to arrive as an attachment that can be printed off.

The design of the template has been attached and must look like this. Font size can be adjusted to suit layout but all boxes / Titles MUST be included.
