I'm looking for someone to modify a fillable Word document I created to IMPROVE on it's DESIGN and FUNCTION. This FORM is used at a FUNERAL HOME to track our CURRENT and UPCOMMING services.
The form is filled out DAILY, as the DATA changes from DAY to DAY.
The FORM is then emailed to our OPERATIONAL staff as well as our ON-CALL answering service. There is a company BRANDED LOGO in the attached file.
The HEADER should be designed as in the attached example. The DATE should default to the CURRENT DATE. The ON-CALL is selected from a DROP DOWN list. This typically defaults to the Location Manager, therefore does not need to be part of the TAB sequence.
Some of fields will come from DATE PICKERS or DROP DOWN LISTS, while others will be FREE FORM, but I would like a format to be suggestged when the MOUSE HOVERS over a field (ie. (Mr./[login to view URL]). LAST NAME, First, Middle, (Nickname)). [See the attached form for examples]
Then the user should be able to TAB from FIELD to FIELD in a SPECIFIC ORDER. As follows...

> Funeral Director Assigned - (REQUIRED Field - Drop Down List - Editable) [Karin, Kaitlin, Ken, Stuart, Paul, Reccia, Clement, Mosey, Other]
> Visitation Date - (Date Picker)
> Time(s) - (Free Form with Suggested Format) example [11:30am - 12:00pm - Private]
> Time(s) - (Free Form with Suggested Format or Blank) [1:00pm - 3:00pm - Public]
> Assigned Room - (REQUIRED field - Drop Down List - Editable) [Chapel, Willowdale, Lansing, Sheppard, Off Site]
> Service Date - (Date Picker)
> Time(s) - (Free Form with Suggested Format) example [3:00pm - 3:30pm - Catholic Service]
> Time(s) - (Free Form with Suggested Format or Blank) [4:00pm - 5:00pm - Burial]
> Assigned Room - (Drop Down List - Editable) [Chapel, Willowdale, Lansing, Sheppard, Off Site]
> Final Disposition/Notes - (Free Form with Suggestions) [Burial, Cremation, Reception to Follow, etc]
The SIZE & LAYOUT of the current form is fine, and allows for 5 ROWS of information to be entered.
The form should auto-update.
For example, if the USER were to DELETE A ROW of data, because it is no longer needed, it would be great if the NEXT ROW simply shifed UP automatically, and then a subsequent BLANK ROW with all the FORMATTING would auto-populate at the BOTTOM of the FORM to take the place of the now deleted row .
That way as the USER deletes the OLDEST row(s), the page will ALWAYS HAVE 5 ROWS, whether they are all populated or not. [See the attached form for examples]
As well, after the 5th ROW of DATA is tabbed off, I would like the form to AUTOMATICALLY create a 2nd (3rd, 4th etc) page with the same HEADERS & FOOTERS carried over, and the same 5 ROWS of fields, all BLANK but the same FORMATTING as PAGE 1.
(Nice to have but not required) - A "PRINT" radio button placed somewhere in the HEADER or FOOTER, that would AUTOMATICALLY print ALL PAGES - DOOUBLE SIDED - on the SHORT EDGE.
(Nice to have but not required) - A "SHARE" radio button placed somewhere in the HEADER or FOOTER, that would AUTOMATICALLY trigger OUTLOOK to OPEN a NEW MESSAGE and ATTACH the .docx as an ATTACHMENT. The USER would then select the ADDRESSEES.
I am a novice user, and have toyed with and tried to convert the form to a PDF or EXCEL spreadsheet, however it was beyond my skill level. HOWEVER if YOU feel that this project would be better handled in either PDF or EXCEL formats I am open to having that conversation and discussing the PROS & CONS of those FORMATS.
