Modify HTML CSS PHP of Website User Login Pages

I have a php user login package that i need to modify a couple of things as per my specific needs.
Changes required:
1) modify fields in mysql database which stores user info
2) change html css of login page with new page i will provide, much implement validation and sanitization of inputs on new page
3) add table to mysql and implement into php code to log every login to system, MUST log ATTEMPTS and SUCCESSFUL logins.
4) modify user sign up page html css and corresponding validation, mysql database fields and php code
only proposals containing the word cat will be considered
