We are looking to have our Signature Sheet form redesign and reworked over. We are looking for a more modern look so fonts would be either Gothic or Montserrat type font families. Our font color is HEX# 174574

What we need to have removed from the current form is any reference to sending the signature in by mail or sending it in by fax.

*** NOTE - we need a clean white area to get a screengrab from so please do not have background images/dots or dashes in this area of the design
If people sign over top of that we have to clean up the signature with an eraser ***

With this form it should be self-explanatory, and we want to make it as easy as possible for our customers to follow

It should contain something as

Step 1 – Enter your Order Number
Step 2 – Sign 3 Times in Black and White
Step 3 Where to Submit Signature

We still need to keep guidelines on best practices and tips, our current form has small dashes or dots around a place for people to sign but we need open white space so that people do not sign on dots or dashes.

This info at the bottom needs to be stripped out or not included that is on the current form
