PSD to HTML/ WordPress Page Template (Must have WP experience)

I require you to convert a PSD into a WordPress Page Template. The attached JPG shows the rendered Page Template — your final work should render the PSD. Of course, the [url removed, login to view] and [url removed, login to view] files render the header and footer.

*You must provide a WordPress Page Template (.php) that will render the entire jpg as described.*

*You must provide the HTML and CSS content for the page content for this page.*

Please see the attached JPG for explanation:

1. The entire Page Template is surrounded by a dark green box.

2. The yellow box shows the content that will appear on every page using this template.

3. The red box shows you Page content that will be different for each page that uses this content. Therefore this should not be hard coded into the Page Template. You must provide CSS and HTML for this section separately.

4. The purple box shows a WordPress Custom Field for the headline.

5. The aqua box shows a WordPress Custom Field for the sub-headline.

6. The yellow box shows a background image that will be set using a WordPress Custom Field.

7. The purple box around the form is telling you that you are allowed to simply use an image slice for the form. I can code the form myself.

*Please provide a detailed quote so I can recognize that you've read my project brief and that you understand it. I need to know you can complete this project competently.*

I would prefer not to provide FTP access. I can provide WordPress access. I've attached the [url removed, login to view] and [url removed, login to view] files so that you can see what is rendered in each so that you can create your Page Template .php file appropriately.
