Need modifications to vb.net mobile application

Hi, we are a small non profit that helps people with intellectual disabilities. We have a vb.net application that allows workers in the field to take notes at client homes and transmitted the notes back to our MSSQL server when the laptop is connected to our network. We need to modify the application to include new fields to be transmitted and we also need the application modified to autogenerate two fields from the previous note for the worker to edit before saving.
To answer some of the questions asked:
This is a desktop application created in Virtual Studio, but had not been updated with the current version of VS ( think the code is one or two revisions ago)
Source code will be provided as well as contact (phone or email) with original developer who can answer any questions about the project. there are some comments in the code itself, but its not documented very well.
All the fields are text or other simple data, including the new fields that need to be added. Some data verification will need to be added to ensure data is collected correctly.
