I need a simple form which allows user to select a file from his/her desktop or mobile device. User can enter comment about the file. User will click on upload button. The file should get uploaded to Amazon AWS S3 The location of file must get saved in database. All of UI interactions should be defined in a "view" file while all uploading and saving in database must be handled in a "controller" file. Please use bootstrap classes in "view" for display. There should be a filter in "view" which allows only image or pdf file to be selected. If I remove this filter later on, any file can be selected for upload
