Prestashop change on product page

On the productpage above the add to cart button I need the file upload to be inserted that is now located in the first tab on the bottom of the product page (see the attachet screenprint)

Also when a customer fills in the Fields :

breedte

Toepassing :

Voor binnen gebruik (zonder beschermend laminaat)

Quantity:

This needs to bee saved when uploading a file, the situation now is thet when a customer fills in these fields and upload a file the fields are empty again.
