PHP Mysql Application

I need a PHP application that will allow me to fill out a page that looks like the PDF attached. The values will be selected from a MySQL database that can be edited with the desired values. Invoice number will auto generate, Customer name will insert into database if it doesn't exist, typing the name will search the database and offer possible matchs on the name and address...the items in the description will be from a drop down list of database values in a table which will populate the unit price, generic customer database name address phone number etc. and another table with item, description,price etc, and option to create a quote that will store the info added to the page and allow me to print a PDF of the information. additionally an option to add images to the quote that are stored in the database as well. *database* Customer Info table auto generate customer id's Quotes table with items quantities etc auto generate invoice numbers, dates, Item table database with items descriptions and cost Invoice table that is the same data as quotes but converts over from quote to invoice. option to upload pictures to quotes and invoices option to browse existing quotes and invoices by customer name,address,date project, etc. completion option to export and save and email PDF with quote or invoice data option to email quote with an entered email with confirmation email I currently don't have a PHP MySQL Server in place but will by the end of the project... create and complete on your separate server and should have an installation script that will prepare the necessary database and pages I can view and give input on your server, then when the project is complete finalize upload completed code and pay.
