Help creating form that's populated from csv data

I am looking for help creating an excel document. My business sets exchange students up with host families for short term visits in the United States. On our website, we have a host family application where they put in various information about their family and home. The end result is just a list of the answers and not something we can give to our students or their families. We created a one page document where we can type/copy and paste the data in but it is a slow and painful process. We are looking for a way to have something like the attached form more of an automated process. We learned it may be possible to import csv data from our online form and that it could be mapped automatically to complete the form for us, which would save us a tremendous amount of time. All the cells that we want auto populated from csv data are highlighted in yellow. Currently we have to manually type all this in and then import pictures of the family on a second page.
