Gravity Forms Routing and Fix Page Script

We are not getting the submissions from our Gravity Form on our site. Also part of the problem is the page the form is on is being overridden by some kind of script. This script loads another page like an iframe into the page so you can't see the form. Please help us to troubleshoot. Please put the word FORM in your bid in all capital letters if you can help. Need this done in the next hour.
