Purpose: 1) setup server and database; 2) create 2 web pages and 1 large form; 3) program email and form Home Page - [login to view URL] User/Pass Page - [login to view URL] Member FORM - [login to view URL] Server & Domain Setup my company on a dedicated server (We will be purchasing the dedicated server next week) Owner will control all user/pass to server and website and FTP access Setup my URL ([login to view URL]) Place text on home page (Global Tech, LLC, JF, 859-476-1627) image only - make it look good Make sure all pages are https Create a new database (PHP / MySQL) Create a new web page USER/PASS that enables users to view FORM ([login to view URL]) Ability to RECOVER user/pass Create a new web page FORM (password access) ([login to view URL]) This FORM will show all the fields for all members to complete (see excel spreadsheet) This FORM contains approx. 130-170 fields (includes radio button select; check boxes; drop-down menus; several select; text fields; Create/design the page in a high-end, professional design look and feel; GREAT USER EXPERIENCE Connect/program this FORM to connect and interact with the new database Ability for members to use phones and tablets to complete the FORM Critical: the database will be used on another domain on Feb. 1, 2019 so design so there is an easy transition There are no other web pages so the entire scope of this project centers around the members completing 130-170 fields Users and Emails Export existing database and import it into the new database (clean up) Create an email that enables members to receive, read, and click on embedded link This email will contain a link to the user/pass page The member will use the TEMP password sent to them The member must immediately create a NEW user/pass with high security The email link and temp password will no longer work Form Reminders We want members to complete ALL fields in one setting but we know that is unrealistic We would like to setup ways to remind, prompt and encourage members to complete the form as quickly as possible We want to create a "counter system" that shows members how many fields there are and how many are still left to complete (i.e., 10 of 175 fields have been completed) We want form controls that indicate whether the member completed a field correctly (i.e. (note: by selecting "other" you must complete the next field "other") When a member SAVES or wants to EXIT, we need to prompt them to continue and also show them how many fields are left over We want as admins to know the status of completion Program a "verify" button indicating the members have reviewed and accepted pre-populated information; otherwise, count as empty field so members don't select verify too quickly
