Build a responsive multi-steps signup form (with payment integration)

- Multi-step form with generic info, then Instagram credential (a "?" tooltip giving more info when you hover it would be nice here), then other text fields $65 - Payment form: we want to create a Chargebee subscription. So far I integrated the Chargebee Checkout at the end of the form, but if we could add payment details directly within the form it would be great. Would need to use the Chargebee API $265 - Redirect to Thank you page $65 - Storing Data: We are using a Google script and some js code to send directly the form results in a google spreadsheet. Maybe it would be best to create a MySQL database for storing the results $265 Only programming work with PHP/MYSQL. I hope there is no design work involved here if yes let me know please. discuss with me so i can see if i can lessen the price for you if you have less budget.
