Build a PHP form and basic mySQL databse

I have a basic 4 page form with roughly 5-10 questions on each page. I have written a rough draft of most of the form pages in html/php with a few (very few) of the variables intact already. I need someone to finish where I left off and write the php handling page to post the information into a MySQL database. I will also need a web page to pull the information out of the database into a form where others can follow up on the responses and amend the database with the status of each response. The questionnaire is going to be embedded in a phpbb3 page with the header and footer wrapper and will utilize the same style sheet as the phpbb forum. I already have this part set up and ready to go but I may want to pull a variable from the phpbb database, such as username, and insert it in the questionnaire as well. It's a pretty basic job but I do not have time to do it myself. I would like to have it completed in the next 10 days or so. I have attached 3 docs that are the first page of the questionnaire. It's pretty basic but this is to get things started so you can see where I am wanting to go. The [login to view URL] calls the [login to view URL] and the [login to view URL] publishes to the database. You can use this format or you can use your own as I don't care as long as it works. I would like it a more detailed as this is just an example of where I want to go with it.
