Build a dynamic content popup window (ninja-forms + wppopupmaker)

We require the submission of a wordpress ninja form to trigger a dynamically populated popup/modal window. This popup will need to contain images and links that dynamically correspond to the form submission data (and maybe some other information). Customers use the ninja form to extend their product warranty, and the popup will invite customers to review their purchase on the correct e-commerce channel where they purchased the product from (product X and retailer Y information will be submitted with the form). For example: 1. Customer extends warranty for product X, 2. Customer purchased the product from retailer Y. 3. Popup shows Thank you-message + Logo from retailer Y, and button to review product X is linked to retailer Y website The difficult part will be linking the products with the appropriate review link on the various e-commerce channels.
