Build a Website

Hi - I am looking for help to build a simple training tracking system for a small manufacturing company. The overly simplified idea is that we need to be able to tell employees that they need training, provide them with a the .pdf training file, and then record their acknowledgement that they read and understood the material. I would like us to stay simple, using php/mysql and a free css like Almost Flat UI. It will primarily be a forms driven site so hopefully nothing overly complex is needed. Four user levels - User, Trainer, Manager, and Admin User Pages: List of Training Due - shows training assigned to employee and due date (click on specific training to launch) Launch and Acknowledge Training - Automatically pops up the .pdf/.doc/.other file for download and includes a password field with submit to acknowledge the training was completed View Completed Training - A simple list of all training and dates completed by that particular employee. Also need to export to .csv file Trainer Page: Upload Completed Training - Sometimes we have in person training that we need to record (employees sign in on a paper sheet). This page will allow the trainer to select a specific training, the employee(s) who attended, and the time/date the training was completed. It also needs to allow the upload of the signed attendance sheet. Manager Page: Reports - As I need to track the training I also need to run reports on it. This will include things like overdue training, training due by date X, training by employee, and training by document (all also need to be able to be .csv exported). Managers can view for their employees, admins can view for all employees Admin Page: Add/Edit Groups - Seeing many of the employees have the same functions and need the same training I would like to be able to create groups such that I can assign the group training once instead of picking out each employee every time. Example groups would be purchasing, manufacturing, shipping, ect. Add/Edit Employees - This page allows the addition and modification of employees. I need to track training history even if individuals are no longer employed (inactive) or on leave and then resume their training if/when they return. Additionally I need to assign a manager, such that emails can go out if individuals are late on training. Add/Revise Training - This will load or update a training topic in the system. This includes basics like document name, number, and revision. Additionally it will define the training frequency (when updated, yearly, 6 months, ect), and groups/individuals who need to be trained on it. Settings - Some generic settings such as timezone, frequency of emailed reports (ie weekly training coming due, overdue, ect), and probably a few other things we come up with as this is being built. I have some hand drawn sketches to show what I want the pages to look like and can share during the bidding process. There are a few areas where I think it would be great to use AJAX to for ease of use to the employees and have those written on the sketches.
