I have very powerful skills when working with Javascript and I can make android apps using Cordova (Android apps with Javascript). I can create a beautiful loading page that will show up to the user while he is waiting and at that time I will create a form depending on the questions and I will send back the questions in an ajax request to the server and If you want I can send additional data like how much time the user took to answer the questions and things like that. I can give your more than 5 hours per week and I'm sure that this app will not need more than 3 hours to be completed but we will know when time comes
