Design A 'Customer Discovery' Survey

What I want to do is use my contact list and email addresses to set up a personalised survey and reach out to each of them to say that I’m creating a new service, and right at the beginning I’m trying to learn about the basics of the day to day hurdles they each face, and that I want to listen to them and discover what their pain points are, what they need most in their businesses, what are they able or willing to spend to get it, I want to make the specific point that I am the guy that will bend over backwards for them and I’m full of great, creative ideas. I want them to come to me with their stories (rather than having to answer any leading questions), I want to find out how do they make their money, how do they sell their own products, what happens next when a buyer wants to buy from them, which supplier do they order them from, how much money do they make from each order, what problems do they have, what sort of packages do they offer, and have they got any orders they want filled or supplied? I want to send out a survey that will work for me as a 'Customer Discovery Project', without trying to hide anything from them or pretend that I am going to offer them something I can’t (I always feel that honesty and transparency is respected by consumers) and I’m going to tell them that the new business plan I am starting up will offer them 2 future services: I intend to ensure that my price range is always 20% lower than my competitors for everything, and I will ask in this survey if they would like to “Join My Club”? 'Price A' is that I will be giving everyone my low prices anyway, but 'Price B' is that those who join ($200 per year joining fee paid upfront, or $18.75 if paid monthly) will get another 10% off all their purchases and other special benefits. I hope that by sending out this survey, I will get a reasonable sample returned to me, but even if only 5% reply, I’ve probably got enough information to determine a business strategy.
