Use webODF to create an ODT from a template and a form data 2

The aim of this project is to use WebODF ([login to view URL]) de generate an ODF from a template. The template is filled with data coming from a a form fields. The result should look like [login to view URL] But the template to be used is the attached file. The form fields are the fields between braces "{}" in the attached file. For example the first field in the form is {DIT} and it must go in 2nd line in the document. please start your proposal with " Hi M. Editor" Even if in the attached file many images are used. only one image must be provided in the final work ; the logo in the top of the document. all other images must be converted to text. The page must also offer the possibily to use formatting menu like in this example : [login to view URL] The delivrable should be : a. one html file with all its dependencies in a zip file that can be unzipped as is on a webserevr and it should work without any other action b. a document explaining step by step how the solution was found
