Implement new design and fix the CSS/JS for my website form.

I have a website form consisting of 6 tabs. It is built with HTML5 and the form submissions are made with AJAX. I want to fix the unresponsive part of forms and implement a new design. Here is the summary of objectives:
1. The top of page has a spacing issue with my nav bar.
2. The design for the tabs needs to be upgraded according to the example I give you.
3. The radio buttons need to be fixed and images need to be added to them instead of text.
4. A final CSS trigger for greying out making all forms uneditable.
5. Add circles to my list.
6. Make the implemented [login to view URL] charts appear (they are not appearing for some reason).
Please note that this task must be done in a downloaded copy of the website. In the end you can give me the updated files and I will upload them to my server.
The developer must start work immediately and deliver the project asap.
