To Develope the required Medical Entry form templates at least 1 to 2, we can discuss more. I have experience with a simular project which was done in MS-Access and had data entry screens build there and after via a button merged into MS-Word templates via data fields. Then it allowed for some additional data entry, Signature and other text was there also. This was a Patient home care nursing wound assessment system for an org. called CCAC.
