Looking to move from paper forms to electronic. I want to use Google forms to collect membership information for a gym. The user will enter. 10+ fields. Then submit.
I would have an iPad at the gym front desk for users to fill the form. Extra to what Google forms offer as standard. I would like the form to:
A. Collect a signature
B. Send a copy of the form and their answers to the user's email. Preferably merged into a nice PDF.
C. On submit they get pushed to a second form from the bank (not our form) to fill out. No work is needed on that form.
Attached is a copy of our current paper-based form. There are a few other fields and 4 forms in total.
