Dear Sir/ Madam, Following is my proposal based on your project detail: I will create a PDF form and protect it with a password. The form will have SUBMIT BUTTON. You can save it on your company server. The employees, authorized to fill it, will access it through the password, update it and then click the submit button. The Submit button will send the form to the designated email. Also, the employee who submitted the form will have to enter his/her name and employee ID or PIN (separate PIN for each authorized employee). So the form will have 2 TEXT FIELDS which will be required to fill to enable an employee to submit the PDF FILLABLE FORM. Otherwise one cannot submit it.
