Dynamic Web Entry Form

I need a web form that draws data from a text file or an excel file to dynamically populate the lookup options of other text fields in the form. The similar type of performance of a form where you select the country, then the form gives you the states, you select the state and the list of cities is provided. A mockup of the web form is attached. Starting from the top of the form there are 6 possible options that select what lookup table is used: Australia ??" Steel Australia ??" Rubber Disk Brazil ??" Steel Brazil ??" Rubber Disk Germany ??" Steel Germany ??" Rubber Disk Once selected a list of possible roll diameters should be presented from the table. The form should default to the first roll diameter. When the roll diameter is selected the list of available bearings should be populated based on which cells have thickness values available. The user will then select a bearing which sets which shaft diameters and which shell thicknesses are available. The “hole?? check box should toggle the display of the hole diameter and hole position input areas. When the fillet is selected to “none?? the fillet radius should not be displayed. With this project the “output?? of the form should be displayed at the bottom of the page. Given the attached excel file as an example. See Attached File!!! ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables): a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
