Wordpress Code Jpeg (Booking system)

Have a template with a form that needs to be coded in Wordpress. Fallowing needs to be done, there are 2 windows with 2 steps. TASK 1 Window 1 - There is 3 options that needs to be chosen by the customer and then a button in bottom to click to get a price shown. The options have an algorithm that you need to make but I provide you a word file with the numbers and logarithms. Window 2 - In this window there is a calendar where the customer chooses the date of the month, the customer should be able to move the calender days 5 weeks forward if he wants. Second he chooses what time of day he wants the service, there are two times, and as you can see there 2 options. In the end its a Booking button where the customer hits it. Then a popup appears where he fills in his information and approved the purchasing rules and a confirmation email gets send to the customer, I will provide what should be in this popup. TASK 2 This should be saved as a widget / interface in wordpress back office, where the admin can edit the bookings and its information. In the attachment you will find the jpeg, any question, please ask, there is no big budget for this so please be reasonable when you bid, and project time should be 3 days or less. The work should be done off site with wordpress so the final result can be presented.
