Service History Book & Maintenance Record

Dear Contest Holder, I am attaching the screenshots of my final work to this contest. The final work includes an easily accessed online interactive form which is asking the user to enter the necessary information regarding the machine service history with the ability to enter description, date, cost and upload supportive documents. The information obtained from the user is then getting stored in an online spreadsheet which creates an online database which then can be used to create a report as required.
